Are you interested in improving health services for people with long term conditions?

Are you interested in improving health services for people with long term conditions?

 

We are looking for people with chronic, long-term conditions (i.e. on-going symptoms or need for treatment) and/or their carers to contribute to a meeting to ensure that the patient information recorded about them meets their needs.  

 

It will involve coming to a workshop and working in small groups to comment on draft documents. we are interested in your experiences as a patient/carer however you will not need to share any personal or private medical information. 

 

Lunch will be provided and we will pay for travel expenses to/from the venue which will be in London. Places are limited and will be allocated on a first-come-first-served basis.  

 

The workshop will be held on 13 November, from 1pm-4pm at the Royal College of Physicians, 11 St Andrew’s Place, London, NW1 4LE.
